Adaptive ClothingAdaptive clothing is specially designed clothing to help make it easier for people who have difficulty dressing. Having clothing that has been adapted can make such a positive difference in a person’s life by giving them the independence to continue to dress themselves or enable carers to dress them with ease and comfort.
Advance Care DirectiveA document completed by an adult who is capable of making decisions, which gives instruction to legally appointed decision makers about future desired medical treatment and is used if the adult becomes unable to make or communicate those decisions
Advance Care PlanningAdvance Care Planning is a process of reflection and communication. It is a time for you to reflect on your values and wishes, and to let people know what kind of health and personal care you would want in the future if you were unable to speak for yourself.

CapabilityBeing able to understand and appreciate a decision: understanding: adult is able to understand the information that is relevant to making a decision appreciation: adult is able to apply that information to their situation Capability is specific to the type of decision and to the particular decision to be made. A determination that an adult is incapable of a particular decision at a particular time does not mean the adult is considered incapable of all future decisions.
Capable AdultAn adult who is: able to understand information about treatment offered, including risks and benefits; and, able to understand consequences of the decision.
Care CardCommon name for the BC Services Card. Each BC resident enrolled with the Medical Services Plan (MSP) is eligible for a BC Services Card with a unique lifetime identifier for health care called a Personal Health Number.
Care PlanThe care plan provides a complete picture of the resident and their care and includes the clinical care path and information that is important to providing resident-centred care. It is developed in collaboration with the resident, family representative, and Care Team and provides details on the resident history as well as the plan for services including treatments, interventions, resident goals, and anticipated outcomes. The care plan incorporates the assessment documented in the RAI-MDS 2.0 and is developed in PointClickCare(PCC). Care plans are part of the resident’s permanent health record.
Care TeamThe group of Broadmead Care staff including physician, RN or LPN, therapists, dietitian, and others who work with the family to create a Care Plan.
Committee of PersonA Court-appointed family member, friend or Public Guardian and Trustee who makes health and personal care decisions for an adult deemed to be incapable.
FamilyAnyone the resident deems to be a family or personal contact.

Health RepresentativeA legal document signed by an adult while they were capable, that appoints a Representative or Representatives to make health decisions on their behalf if incapable.
Incapable AdultAn adult who: is unable to understand information about treatment offered, including risks and benefits; or, who may be temporarily incapable due to unconsciousness, extreme pain, alcohol or drugs, cognitive loss or dementia.
Island HealthThe health authority for Vancouver Island and the Gulf Islands. It is one geographical jurisdiction of BC Health.
LPNLicensed Practical Nurse
Maintenance AccountAn account in the resident’s name through which monthly rent is paid.
Monthly chargeThe amount of monthly rent payable by a resident. This is determined by an Island Health formula and is different for each person.
Personal SnapshotThe Personal Snapshot provides a quick reference summary of the daily personal care needs of the resident in one concise easy-to-follow document.
PhilanthropyWith its anthro- root, philanthropy means literally ‘love of mankind”’.Thus, philanthropy is giving money for a purpose or cause that benefits people you care about. Read more about philanthropy on page X.
Power of AttorneyA Power of Attorney is a legal document. It gives one person, or more than one person, the authority to manage someone else’s money and property.
Primary Family ContactThis should be the person who is either officially appointed as Committee of Person or Health Representative for the resident, or who will make healthcare decisions regarding the person’s care (if he or she is unable to make their own decisions).

Temporary Substitute Decision MakerWhen there is no Committee of Person or Representation Agreement, a person who has the right to give, withhold, or revoke consent to health care for an adult who is incapable of making those decisions for him or herself
Trust AccountAn account in the resident’s name, held in trust at the Care Home, used for special activities and events. The resident can withdraw cash through this account.
Veterans Service CardThe Veteran’s Service Card provides a tangible symbol of recognition for former members, and encourages an enduring affiliation with the Canadian Armed Forces. All serving Canadian Armed Forces members and Veterans are encouraged to register for a MY VAC Account with Veterans Affairs Canada.
